An End of Life Specialist provides non-medical support, guidance, education, and companionship to people living with a life-limiting diagnosis or who are approaching the end of life, as well as to those closest to them.
This role is different from that of a carer or healthcare professional. An End of Life Specialist offers continuity of support and a broad range of services tailored to individual needs, values, and circumstances.

Supporting clients to reflect on and choose an end of life that is meaningful and aligned with their values, including religious, spiritual, cultural, and personal preferences.
Assisting in creating an end of life that is as intimate and peaceful as possible, whether at home, in hospice, or in a residential care setting.

Supporting and guiding clients of any age to plan, discuss, and record their end-of-life preferences.
Assisting with completing an Advance Care Plan or Advance Care Directive.

Acting as an advocate by liaising with healthcare professionals, carers, family members, and support services throughout all stages of the end-of-life transition.
Accompanying individuals to medical appointments or treatments, taking notes, asking questions, clarifying information and relaying information as required.

Supporting clients to live as fully and comfortably as possible in the weeks, months, or days prior to death. This may include:
Organising support services in the home or assisting with transitions to respite care or residential aged care.
Decluttering of home.
Creating legacy items such as photo memory books or slideshows.
Supporting the completion of meaningful goals or “bucket list” wishes.

Independent, unbiased guidance to help you understand your options and choose a funeral home that aligns with your wishes, values and financial needs.
Creating and leading a personalised, meaningful funeral or memorial service as a celebrant.
Organising and leading a living wake or farewell gathering.
Producing a photo tribute to be played at a funeral.

Providing compassionate grief and bereavement support for individuals and families before and after death.
